    Patient Relationship Officer and Care Manager

    Reports To: BUSINESS DEVELOPMENT AND MARKETING MANAGER

    JOB PURPOSE

    Reporting to the Marketing and Business Development Manager, the candidate will be responsible for ensuring a positive patient experience by managing patient interactions, addressing concerns, and coordinating comprehensive care plans. This role involves acting as a liaison between patients, healthcare providers, and administrative staff to ensure the highest standards of patient care and satisfaction.

    MAIN DUTIES AND RESPONSIBILITIES

    Patient Relationship Management:

    • Serve as the primary point of contact for patients and their families throughout their care journey.
    • Address patient inquiries, concerns, and complaints in a timely and professional manner.
    • Build and maintain strong relationships with patients, fostering trust and ensuring a positive experience.
    • Monitor patient satisfaction levels and implement strategies to improve the overall patient experience.

    Care Coordination:

    • Develop, implement, and monitor individualized care plans in collaboration with healthcare providers.
    • Coordinate patient care activities, including scheduling appointments, follow-ups, and coordinating with specialists.
    • Ensure continuity of care by keeping patients informed about their treatment plans and any changes in their care.

    Communication:

    • Act as a liaison between patients, families, healthcare providers, and other hospital departments.
    • Facilitate clear and effective communication between all parties involved in patient care.
    • Provide patients and families with information about available services, treatment options, and support resources.

    Documentation and Reporting:

    • Maintain accurate and up-to-date records of patient interactions, care plans, and outcomes.
    • Prepare reports on patient satisfaction, care outcomes, and other relevant metrics for management review.
    • Ensure all patient information is handled confidentially and in compliance with relevant laws and regulations.

    Patient Advocacy:

    • Advocate for patients' needs and rights within the healthcare system.
    • Assist patients in understanding their medical conditions, treatment options, and healthcare rights.
    • Help patients navigate the healthcare system and access the necessary resources and support.

    Quality Improvement:

    • Participate in quality improvement initiatives aimed at enhancing patient care and experience.
    • dentify areas for improvement in patient services and collaborate with other departments to implement changes.
    • Monitor and evaluate the effectiveness of care management programs and make adjustments as needed.

    Education and Training:

    • Educate patients and families about their healthcare options, treatment plans, and post-care instructions.
    • Provide training and support to healthcare staff on patient relationship management and care coordination best practices

    MINIMUM REQUIREMENTS/ QUALIFICATIONS

    • Bachelor’s degree in nursing (BSN) or equivalent. A master’s degree in healthcare management or a related field is preferred.
    • Registered Nurse (RN) with a valid practicing license.
    • At least 5 years of experience in nursing and healthcare management, with a focus on patient care and customer service.
    • Customer Care training.
    • Strong leadership and team management skills.
    • Excellent communication and people skills.
    • Ability to manage stressful situations and resolve conflicts effectively.
    • Proficiency in healthcare management software and systems.
    • Knowledge of regulatory requirements and standards in healthcare.

    Midwife/ Maternity Nurse

    Reports To:  NURSE-IN-CHARGE

    JOB PURPOSE

    Providing superior nursing care in management of prenatal, labor and delivery, and postpartum care of patients. 

    MAIN DUTIES AND RESPONSIBILITIES.

    • Delivering professional and high standard of nursing care within maternity by conducting regular checkups, examination during pregnancy, monitoring the health of the mother and the developing baby
    • Providing guidance on nutrition, exercise, and overall wellbeing of the mother to promote a healthy pregnancy
    • Assist and support women during labor and delivery, ensure safe and comfortable environment, monitor hemodynamic changes on both mother and child in utero and post-delivery and provide post-partum care.
    • Timely response to emergency within the department
    • Monitor and ensure clients satisfaction and proactively manage their complaints as they arise.
    • Review and ensure that nursing care plans are scientific, evidence based and address the priority needs of mother and child.
    • Liaise with doctors and other care providers regarding patients’ Plan of Care and Care solutions with emphasis on a team-based problem-solving approach.
    • Provide unified approach to patient care.
    • Accountable for ward inventory of all assets in the section (equipment, beds, surgical, drugs, solutions etc.) and account for stocks of consumables and ordered and utilized in the department.
    • Ensuring proper and timely documentation, maintaining proper records, confidentiality, and handing over between shifts at all times in maternity and any other ward where the nurse is deployed
    • Participate in development and ensure implementation of organizational and departmental policies and procedures (SOPs) and quality improvement plans in the section including infection prevention in maternity.
    • Ensuring all equipment in the section are in working condition at all times through compliance with the PPM and reporting promptly the faulty equipment including but not limited to quality reports, Ministry of health reports etc.
    • Control the departmental budget within the agreed parameters by ensuring efficient utilization of resources and proper charging with keen attention to billing instructions and monitoring of exclusions
    • Participate in business development and marketing activities and follow up on customer feedback.
    • Ensure safe and clean environment for staff patient/clients and visitors compliance with policies and protocols including health and safety, risk management and critical incident reporting.
    • Any other duty as assigned by the supervisor in line with the job description.

    MINIMUM REQUIREMENTS/ QUALIFICATIONS

    • Diploma in Kenya registered community Health Nurse/ Midwife (KRCHN/M) from a recognized institution.
    • Bachelor’s degree in Midwifery will be an added advantage.
    • A valid Nursing council license
    • Updated BLS, ACLS & EMONC training certifications.
    • At least 3 years’ experience in medical-surgical ward.
    • Knowledge in computer applications

    Medical/ Surgical Nurse

    Reports To:  NURSE-IN-CHARGE

    JOB PURPOSE

    Providing high standard of nursing care to all patients in line with the Hospital’s policies and procedures and achieving patient safety goals.

    MAIN DUTIES AND RESPONSIBILITIES.

    • Deliver direct patient care including but not limited to assessment, planning, implementation and evaluation of nursing interventions through professionalism and high standard of nursing care within the department.
    • Advocate for patient needs, rights and preferences ensuring a patient centered approach to care.
    • Serve as an expert resource person providing expert nursing care and demonstrate advanced clinical skills and knowledge in nursing practice.
    • Engage on a daily basis with doctors and other care providers regarding patients’ plan of care and care solutions with emphasis on a Team-based problem-solving approach to unified approach to patient care.
    • Collaborate with interdisciplinary health care teams to coordinate the delivery of comprehensive care.
    • Facilitate effective communication between healthcare professionals, patients, and their families to achieve optimal patient outcomes.
    • Participate in quality improvement initiatives and ensure adherence to established standards, policies and procedures. Monitor and evaluate quality of care provided, identify areas of improvement and implement corrective actions as necessary.
    • Administering treatment to patients a prescribed.
    • Carry out patients’ proper orientation within the section.
    • Relay accurate information to clinical staff on a patient’s condition.
    • Responding to and reporting any abnormal recordings to the person in charge.
    • Respond to emergencies when they arise.
    • Labeling and dispatching specimens to laboratory promptly and safely.
    • Perform routine nursing duties, prepare for and clear the ward before admission and after discharge.
    • Ensure patients basic needs are met (feeding, bathing, change of linen etc.)
    • Provide health education to patients to promote health.
    • Proper documentation, maintaining proper records, confidentiality and handing over between shifts at all times.
    • Identify and address potential risks and hazards in the care environment. Implement preventive measures and promote patient safety initiatives.
    • Carry out special outpatient clinic activities (maternal, well baby, OPD).
    • Any other duty as assigned by the supervisor in the line with the job description.

    MINIMUM REQUIREMENTS/ QUALIFICATIONS

    • Diploma in nursing from a recognized institution. A degree in nursing will be an added advantage.
